Lunch Order Form
Monthly Lunch Calendar
Please read our outline carefully.
- Calendar menus will be sent home monthly as a courtesy. These are subject to change. The actual meal is listed on the lunch order form which is sent out bi-weekly (short weeks and holidays are the exception). Menus will also be available on the school website.
- Order forms are due back on Thursday morning unless otherwise stated on the form. Please check the form for the actual due date.
- All order forms must be accompanied by cash or check – there is an $8.00 surcharge for all returned checks.
- If you have more than one child ordering, please put all order forms and money together in one envelope and send into school with one child.
- Please write your child’s/children’s name(s) and homeroom number(s) on the outside of the envelope.
- Extra order forms are kept in the school office and the kitchen and found on the website – do not write orders on any other papers.
- Please do not include lunch money in with any other classroom fees such as milk or field trips.
- If your child is absent on a day that he/she ordered lunch, you must fill out the credit section on the next order form you receive. Your child does not automatically get a credit nor do they receive that meal the next time it is offered.
- Any child without a lunch must notify the kitchen by 9:00 am. The child will be offered the choice of bologna, jelly or cheese sandwich. IOU’s are given to any child without a lunch and sent home the same day. Please return the slip with the payment the following school day.
