Hello to all Our Lady of Fatima School families
My name is Adele Brett and I am the school nurse here at Our Lady of Fatima five days a week. I am pleased to have the opportunity to provide a safe and healthy environment for your child (ren) while they're in school.
Please assist me by ensuring that all health records are completed and accurate for your child(ren). I begin each year by reviewing all records. I will contact you if there is any additional information that needs to be included.
Communication with parents/guardians plays a key role in my job. In order to best help your child I need to know if your child has any specific medical problems. These may include allergies, medications, and chronic conditions.
According to Delaware laws, the Department of Education regulations, and the Diocese of Wilmington, all children entering school for the first time are required to have proof on file of the following:
Physical
Kindergarten, First grade (if not done in Kindergarten or Pre-K) Fourth grade and all new entrants from out of state are required to have a physical.
Immunizations
5 doses of DtaP or DTP vaccine (unless 4th dose was given after the 4th birthday)
4 doses of IPV or OPV (unless 3rd dose was given after the 4th birthday)
2 doses of MMR vaccine (first dose at the age of 12 months, second dose after the 4th birthday)
3 doses of Hepatitis B vaccine
2 doses of Varicella vaccine or documentation of disease from a healthcare provider
Tuberculosis
Results of Mantoux skin test completed within the past 12 months or a Pediatric TB Risk Assessment as recommended by the Delaware Division of Public Health.
Blood Lead test
Documentation for children age 5 or younger - usually done at age 1
Screenings
During the school year, vision, hearing and scoliosis (curvature of the spine) screenings will be performed for the purpose of detecting children in need of further evaluation. The following grades will be screened:
- Vision - kindergarten through 8th grade.
- Hearing - grades 1, 3, 5 and 7
- Scoliosis - grades 5, 6 and 7
- Height and Weight - grades 1, 3, 5 and 7
You will be notified if the results of any screening require additional testing by your physician.
Medications
Medications can be given to your child/children while they're at school. You'll be requested to sign a medication authorization form if your child must take prescription medication during the school day. The medication must come to school in its original container with the prescription label attached and will be kept in my office.
Nonprescription medication may also be given to your child. However, a nonprescription medication form must be completed and signed by you and your child's physician.
During the School Year
The school year brings some illnesses that are spread easily from child to child. Here are some hints to keep your child healthy:
- Handwashing, handwashing - the first line of defense.
- Plenty of rest - 9 to 10 hours sleep a night to perform at their best
- Healthy eating and exercise - encourage your children to eat five fruits/vegetables per day and eat a nutritious breakfast. Encourage your child to have a least an hour of physical activity per day.
Please keep your child home if they have the following:
- A temperature of 100°F or higher. Your child should be fever free without medication for 24 hours before returning to school.
- Vomiting or diarrhea more than once in the past 24 hours. They should be able to keep down both liquids and solid foods before returning.
- If your child develops strep throat or conjunctivitis (pink eye), they will need to be on an antibiotic for 24 hours before returning to school.
